The Complaint, History, Exam, Assessment Grid page shows us SOA forms for each species in VETport and also shows their status.

1) List of Species in VETport

2) Status

3) Click on ADD/Edit SOA forms.

You can add/modify Assessement forms on this page.

1) Select the species.

2) Search for the assessment form by name

3) Search for assessment forms by System or Category

4) Add a new Assessment Form

5) Edit the existing assessment form.

1) Edit the name of the Existing Assessment

2) Change Status to Active/Inactive

3) Change the System

4) Select type from Disease/Sign/None

5) Save changes by clicking on Modify.
